How to about the remote village address in case of Bangladesh

Hi, all
I need a little help from you guys. I am from Bangladesh. In order to register for a new card I need to signup and provide address where that intended card to be sent. Now the problem is my remote village address does not have the formatting like the street address needed in the signup form. e.g signup form needs pattern like 81/12 uttar jatrabari that is correct street address but my remote local address is like C/O Abul Hossain, village:Dasdona, Post Office:Narndi, Police Station:Monohardi, District:Narsingdi. So in this case what can I do please help.
