Getting error of one or more Incorrect details while adding new bank account.

Hello everyone, I am facing an issue whenever I try to add "withdraw to bank" service, i filled the form with correct information but I get an error "incorrect details" though I am sure I am entering correct details. every time I attempt more than 3 times the account get locked temporarily. Any help would be appreciated. thanks
