An Card Reissue Problem

When I try to fill the Form when Ordering new card before my Old one Expires, I make sure I will give the Correct information, But Once I click Reissue Card after I finish filling the Forms, I get the Error Message " Wrong Details " and I did that multiple times and checked the details multiple times but nothing works..

i wish i get support as soon as possible or I wanna say before 2020 when my card will be Expired and I really need it in that period, so again, please help, thanks
