Question regarding Fiverr withdrawals & expired card

I have registered my account in 2016. My bank details, phone, National ID all verified. But after a while, I have stopped using Payoneer because most of my clients were local & I can receive direct payments. But recently I have joined Fiverr & have some earnings which I want to withdraw using Payoneer.
The first thing which I have noticed is my USD prepaid card was expired last year. Since I don't have enough balance in my account they have asked to deposit at least 30$ to activate the renewal option. So I have withdrawal an amount of 68$, while it's pending I got an email from Payoneer demanding for additional information. I have seen there were 2 things required my address verification & "Global payment service questionnaire". I have submitted my e-statement today for address verification & it is pending might take 3-5 days. But my question is do I need to fill out "Global payment service questionnaire" as well? because I'm only working on Fiverr nowadays & don't have any plan to receive any payments from international clients.
About expired card well I have the option activated to order now! but the problem is I have lost that card few months ago & don't remember the 16 digit numbers. How can I get a new card in case if I don't have 16 digit number?
Any guidelines are welcome from the community or support team, I'm old member of Payoneer but still pretty new to this platform :)
